We request that you send us the following records, so that we can do a preliminary assessment to determine whether or not seed brachytherapy is the best option for you.

Biopsy Reports
Your PSA Blood Test Results
Any Physician Notes Addressing Your Cancer
X-Rays and Bone/CT/MRI Scans (if done)
Prostate Ultrasound Examinations (if applicable)
A Copy of Your Insurance Card

If the decision is made by you and one of our doctors to proceed, an appointment will be made for your prostate volume study and consultation. Family members and loved ones are welcome to accompany you.
